Job summary

Creative Solutions in Healthcare - West is seeking a Per Diem Physical Therapist to join our in-house team serving residents with person-centered care. You will assess, plan, and administer rehabilitation therapies within a SNF/long-term care setting, ensuring the plan of care is followed and documentation is complete.

New grads are welcome. A Texas PT license is required, with flexible schedules and competitive compensation.

Qualifications

• Active Texas license in Physical Therapy is required.
• In good standing with all regulatory agencies.
• Experience in SNF/long-term care is preferred.

Responsibilities

• Consult with patients to learn their physical condition.
• Develop and implement treatment plans and rehabilitation programs.
• Monitor patient progress and adjust plans as needed with IDT.
• Ensure documentation is thorough and compliant with regulations.
• Collaborate with interdisciplinary team to ensure quality care.
• Attend required meetings and provide education to patients and families.
