Job summary

Creative Solutions In HealthCare is seeking a Per Diem Physical Therapist to join our in-house rehab team serving residents with person-centered care. The role requires an active Texas PT license and experience in SNF/long term care.

Per diem flexible schedules, competitive compensation, and the opportunity for new grads to apply. You will assess, develop and implement rehabilitation plans, document in Net Health, and collaborate with the interdisciplinary team.

Qualifications

• Active Texas license in Physical Therapy.
• In good standing with regulatory boards.

Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans and rehabilitation programs.
• Administering PT treatments to relieve pain and restore function.
• Documenting thoroughly and timely per regulations.
• Collaborating with the interdisciplinary team to ensure quality care.
