Job summary

Somerset Post Acute in El Cajon, CA is seeking a dedicated PRN Physical Therapist to join our in-house therapy team and help drive high-quality patient care. You will screen and evaluate patients with cognitive and ADL impairments and develop evidence-based plans of care aligned with regulatory standards.

This position offers a flexible PRN schedule within a supportive leadership environment, with competitive pay of $50-$60 per hour based on experience, and opportunities to influence

Qualifications

• Licensed Physical Therapist.
• Open to PTs of all experience levels.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL impairments and develop plan of care following regulatory and clinical standards.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled and medically necessary treatment within regulatory and clinical requirements.
• Administer procedures as part of the rehabilitation plan including ADL retraining and therapeutic activities.
• Assess progress and adjust treatments in medical records; evaluate outcomes and refer as needed.
• Provide counseling to patients and families; communicate progress with supervision and health team.
• Prepare comprehensive discharge summaries per clinical requirements.
