Responsibilities

• Evaluating, creating individualized treatment plans and celebrating patient progress
• Maintain accurate and timely documentation according to facility guidelines
• Help residents regain strength, mobility, confidence, and independence
• Work with an interdisciplinary team that collaborates effectively
• Educate residents and families while building meaningful relationships
• Bring positive energy and compassion to work daily
