Job summary

Yanceyville Rehabilitation And Healthcare Center in Yanceyville, NC is seeking a part-time PRN Physical Therapist to join our long-term care team. You will perform comprehensive PT evaluations, develop treatment plans, and deliver therapy while coordinating with physicians, nursing staff, and families.

You will document progress, manage patient care plans, participate in meetings, and ensure ethical billing and compliance with facility policies. A current PT license in the state is required.

Qualifications

• Graduate degree in Physical Therapy (DPT/MS/MA/B.S. as recognized) by the APTA.
• Current license/registration to practice physical therapy in the state.

Responsibilities

• Provide comprehensive PT evaluations based on physician orders.
• Document findings on standardized forms with accuracy.
• Develop and implement treatment plans with short and long-term goals, frequency, and modalities.
• Obtain physician orders for evaluations, treatments, re-certifications, and discharge.
• Deliver treatments using appropriate modalities and adjust as needed with safety.
• Document daily participation and weekly progress.
• Update treatment plans and discharge summaries as indicated.
• Document caregiver or discharge in-services when required.
• Complete all necessary facility documentation.
• Contribute to MDS-related care plan information as required.
• Attend rehab meetings and participate in interdisciplinary team discussions.
• Schedule patient treatments and perform patient screens as needed.
• Complete billing for physical therapy services in an accurate and ethical manner.
• Cover other therapists’ caseload during absences as necessary.
• Supervise junior staff or students and co-sign notes where required.
• Maintain professional relationships with staff, patients, and families.
• Provide in-service education on patient care to rehab staff and hospital teams.
• Comply with facility policies, confidentiality, and Resident Rights.
• Report to work on time and uphold productivity and professional standards.
• Adhere to HR policies regarding breaks, sick leave, and time off.

Education

Graduate with a DPT/PhD/MS in Physical Therapy recognized by the APTA; B.S./MS/MA also acceptable

Active state licensure/registration
