Job summary

Yanceyville Rehabilitation And Healthcare Center in Yanceyville, NC is seeking a Physical Therapist (PRN) to perform comprehensive evaluations and develop treatment plans based on MD orders. You will document findings, set goals, and coordinate orders for evaluations, treatments, re-certifications and discharges.

The role requires delivering patient care using modalities, updating plans, and maintaining thorough documentation while complying with facility policies and HIPAA.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license and/or registration in the state of North Carolina (as applicable).

Responsibilities

• Provide comprehensive PT evaluation based on physician orders.
• Document findings on standardized forms and adhere to guidelines.
• Develop comprehensive treatment plans with long/short-term goals and modalities.
• Ensure MD orders are obtained for evaluations, treatments, and discharges.
• Provide care using modalities and adjust treatment with patient progress; maintain documentation.
