Job summary

AdvantageCare Rehabilitation is seeking a PRN Physical Therapist to join Indian Lake Rehabilitation Center in Lakeview, Ohio. You will provide weekday PRN coverage, evaluating and treating adults and geriatric patients while collaborating with an interdisciplinary rehab team.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Ohio Physical Therapist license or eligibility.
• Strong clinical and communication skills.
• Patient-centered approach to rehabilitation.
• Ability to work collaboratively with the interdisciplinary team.
• Dependable availability for agreed-upon PRN coverage.

Responsibilities

• Perform comprehensive Physical Therapy evaluations.
• Develop individualized plans of care and functional goals.
• Address strength, balance, endurance and mobility.
• Provide gait and transfer training.
• Improve safety and reduce fall risk.
• Provide therapeutic exercise and functional activities.
• Educate patients, families and caregivers.
• Collaborate with PTAs and the interdisciplinary care team.
• Monitor progress and modify treatment plans as needed.
• Complete accurate and timely clinical documentation.
