Job summary

McIntosh Care & Rehabilitation in Longmont, CO is a post-acute care facility offering in-house therapy and geriatric services. We are currently seeking a PRN Physical Therapist for our rehab program, with a stable, supportive team and a values-driven culture.

The position offers a flexible PRN schedule and a pay range of $40–$55 per hour, depending on experience. Open to PTs at all experience levels, including new grads, to join our CAPLICO-focused team.

Qualifications

• License to practice physical therapy in Colorado is required.
• Open to PTs at all experience levels; new grads welcome.

Responsibilities

• Screens/evaluate patients with cognitive and/or ADL physical functioning disorders and develop plan of care.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled, and medically necessary treatment per regulatory requirements.
• Administer procedures as part of the rehabilitation plan (ADL retraining, exercises, modalities, devices).
• Assess progress and adjust treatments in medical records.
• Evaluate outcomes and modify services accordingly; refer as needed.
• Provide counseling to patients, families, and caregivers regarding physical disorders.
• Communicate progress with supervisor and health team members.
• Provide discharge summaries per regulatory requirements.
