Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to join a therapist-led team and help scale a growing network across the country.

You will evaluate, develop, and implement individualized treatment plans in coordination with other disciplines, document patient progress, and educate patients and families on home exercise programs and injury prevention.

Schedule is PRN daytime with weekdays and weekends at Heritagespring - West Chester OH.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
