Job summary

Evolve Therapy Services, LLC is seeking a licensed Physical Therapist for an in-house Skilled Nursing Facility in Manitowoc, WI. This PRN role focuses on delivering quality therapy services and collaborating with an interdisciplinary team to support resident well‑being.

You will evaluate residents, develop treatment plans, ensure CMS-compliant documentation in NetHealth EMR, and participate in ongoing education to maintain licensure and deliver evidence-based care.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and departmental procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Develop and implement treatment plans with short and long-term goals.
• Communicate evaluation findings and treatment recommendations to residents, families, and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
