Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in South Range, WI. This PRN role focuses on delivering high-quality PT services to residents per standards.

You will evaluate residents, develop treatment plans, document in NetHealth EMR, and collaborate with an interdisciplinary team. Requires BS in PT and active WI license; ongoing CE is encouraged.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice (WI).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents according to standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up.
• Conduct regular screenings to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Interpret and communicate findings to residents, families, and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
