Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
• Maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standard practices and departmental procedures.
• Document and bill per CMS and company requirements.
• Evaluate and re-evaluate residents; develop and adjust treatment plans.
• Screen residents to determine need for intervention and treatment.
• Communicate findings and plans with residents, families, and the care team.
• Complete compliant, medically necessary documentation in NetHealth EMR.
• Perform other duties to support resident care and organization goals.
