Job summary

Life Care Centers of America is seeking a Physical Therapist to provide direct rehab services in Skilled Nursing Facilities. The role requires following physician-referred therapy plans, establishing measurable goals, and collaborating with an interdisciplinary team to support patient progress in a patient-centered environment.

Qualified graduates with active state licensure and CPR certification are encouraged to apply; 1 year of experience is preferred, but new grads are welcome.

Qualifications

• Graduate of an accredited program in physical therapy and currently licensed in the applicable State.
• CPR certification on hire or during orientation; maintain current CPR.
• New grads welcome to apply; 1 year experience preferred.

Responsibilities

• Follow and adjust PT treatment plans per physician referral.
• Establish and modify functional goals for patients.
• Supervise and evaluate PTAs, Rehab Aides, and students.
• Maintain documentation and charting; use appropriate therapy software.
