Phoenix Home Care and Hospice seeks a dedicated Physical Therapist Assistant to deliver in-home patient care under the supervision of a licensed PT. You will help patients achieve mobility goals, promote independence, and maintain an active lifestyle in Bolivar, MO and surrounding areas.

Qualifikationen

• Physical Therapist Assistant certification for MO is required.
• Home Health experience preferred but not required.
• Must pass drug test, background check, and physical.
• Demonstrates ability to collaborate across multidisciplinary teams.

Aufgaben

• Provides PTA assigned services to patients in the home setting under the supervision of a licensed PT.
• Participates in communication and case conferences to promote continuity and quality of care.
• Exhibits sound judgment and technical skills for treatments per agency protocols.
• Maintains compliance with the Agency Code of Ethics.
• Ensures compliance with agency standards, policies, state and federal regulations.
• Practices safety, infection control, and environmental measures.
• Performs other duties as assigned by Supervisor.
