Responsibilities and Duties:

1. Implement plan of care in consultation with physical therapist and other care team members.
2. Communicate findings or changes in regard to patients back to the physical therapist and other care team members.
3. Educate patients and their families in basic home safety and medical equipment safety.
4. Solution-driven, self-motivated, and inspiring to patients to reach their goals.
5. Work independently without direct oversight.
6. Able to discern when to call for support and communicate challenges.
7. Familiar and comfortable with technology.

Qualifications and Skills:

1. Associate Degree in Physical Therapy.
2. Current Certification in BSL/CPR.
3. Home Health Experience.
4. Valid Texas Driver's License and Automobile Insurance.
5. Knowledge of WellSky EMR software is a plus.
