Functional Pathways is seeking a PRN Physical Therapist for Westwood Ridge by Vivie in West St. Paul, MN and travel to nearby Vivie communities in the Metro Minneapolis/Saint Paul area.

The PT will screen, evaluate, and revise care plans under physician supervision, ensuring high-quality resident care and collaboration with a multidisciplinary team. Active state licensure and strong communication are required.

Qualifications

• Bachelor's, master's or Doctorate in physical therapy from an accredited program.
• Active state license required.
• Certification and licensure requirements as established by state and national board associations.
• Excellent organizational and time management skills.
• Excellent written and verbal communication skills.
• Ability to supervise Physical Therapy Assistants.
• Completion of all required screenings, certifications, and licensure. Screening requirements may vary by location.

Job Summary: The Physical Therapist shall screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care as supervised by the physician.
