Overview

PRN Physical Therapist role at Cedarhurst Senior Living. This description reflects a service-minded, patient-focused position within a senior living environment.

As a Physical Therapist, you will connect with and positively impact the lives of our patients daily, promoting physical, personal, and emotional well-being and working with the team to help patients regain maximum self-care and independence when possible.

Responsibilities
  • Providing direct patient care activities to patients as directed by physician orders, including assessment, treatment planning, implementation, evaluation, and instruction/supervision of physical therapy practice within the Standards of Practice for Physical Therapy and in compliance with laws, regulations, and agency policies.
  • Performing the initial and/or comprehensive physical therapy evaluation, identifying the primary focus of care, and developing an individualized plan of care in collaboration with the interprofessional team, patient, family, or caregiver.
  • Educating patients and their families or caregivers about managing illness or injury, including home care needs, home safety, and use of assistive and adaptive devices.
  • Participating in care coordination and discharge planning activities and acting as a resource to other health care personnel in meeting patient needs.
  • Participating in activities that promote patient safety, quality, and regulatory compliance.
  • Engaging in professional development and lifelong learning activities.
  • Supervising PTAs.
  • Performing other responsibilities as assigned.
Qualifications
  • Bachelor's, Master’s, or Doctorate degree in Physical Therapy
  • Current Physical Therapy license in the state(s) of practice
  • Current Basic Life Support/CPR certification
  • Current and valid driver’s license, reliable transportation, and current liability auto insurance
  • Ability to meet physical demands of the job, including seeing, speaking, touching, walking, standing, bending, reaching, crouching/kneeling or crawling, and the ability to lift/move up to 50 lbs independently and up to 200 lbs with assistance
  • Ability to work well with others, take direction from management, and take initiative to ensure patient needs are met and comfort in living environment
  • Self-motivated with accountability for results
