Job summary

Legacy Healthcare Services, Inc. is seeking a Physical Therapist (PRN) to join our on-site team in Illinois. This role emphasizes one-on-one patient care, individualized treatment plans, and collaboration with a coordinated care team to improve residents’ quality of life.

Qualifications

• Degree in area of discipline.
• Current Physical Therapy license in state of practice, or ability to obtain prior to beginning employment.
• Excellent interpersonal, fiscal, and clinical skills.
• Computer proficiency.

Responsibilities

• Provide rehabilitative services to residents under specific medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to share and coordinate the plan of care for patients.
• Treat residents one on one, not in a group setting, to ensure individualized attention through private sessions.
• Customize treatment sessions to meet each resident’s needs.

Pay Range
