Job summary

Holladay Park Plaza in Portland, OR is seeking a PRN Physical Therapist to join our therapy team. The Physical Therapist shall screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care under physician supervision.

This role supports compassionate rehabilitation within a retirement community focused on dignity and resident well-being.

Qualifications

• Bachelor’s, Master’s or Doctorate degree in physical therapy from an accredited program.
• Active state license required and licensure requirements as per state/national boards.
• Excellent organizational and time management skills.
• Excellent written and verbal communication skills.
• Ability to supervise Physical Therapy Assistants and apply policies and procedures.

Responsibilities

• Screen, evaluate and re-evaluate patients and contribute to discharge planning under physician supervision.
• Make revisions to established plans of care as needed.

Job Summary

The Physical Therapist shall screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care as supervised by the physician.
