Responsibilities

  • Patient Assessment: Evaluate patients’ physical conditions, mobility, and functional abilities to develop treatment plans.
  • Treatment Planning: Create and implement personalized therapy programs addressing goals and mobility.
  • Therapeutic Interventions: Administer exercises, manual therapy, and modalities to support rehabilitation.
  • Progress Monitoring: Regularly assess progress and adjust plans to optimize outcomes.
  • Patient Education: Educate patients and families on exercises, safety, and home care strategies.
  • Collaboration: Work with physicians, nurses, and other professionals for coordinated care.
  • Documentation: Maintain accurate evaluations, plans, progress notes, and discharge summaries.

Responsibilities

  • Therapeutic Interventions: Administer therapeutic exercises, manual therapy techniques, and modalities to help patients achieve their rehabilitation goals.
  • Progress Monitoring: Regularly assess and document patient progress, adjusting treatment plans as necessary to optimize outcomes.
  • Patient Education: Educate patients and their families on exercise techniques, safety measures, and home care strategies to support ongoing recovery and maintenance.
  • Collaboration: Work closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and address any issues that may impact patient progress.
  • Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries in compliance with facility policies and regulatory requirements.
  • Compliance: Adhere to all safety and infection control protocols, as well as facility policies and procedures, to ensure a safe and effective therapeutic environment.
Qualifications
  • Education: Doctoral or Master’s degree in Physical Therapy from an accredited institution.
  • Licensure: Current state licensure as a Physical Therapist.
  • Experience: Previous experience in a skilled nursing or long-term care setting preferred, but not required.
