Qualifications

• Must hold a current Texas PT License and CPR Certification.

PRN Physical Therapist will provide direct patient care to homebound patients. Pay will be per visit.

Responsibilities and Duties:

1. Develops and revises plan of care in consultation with physician and other care team members.
2. Provides therapeutic treatments and evaluates equipment needs as applicable.
3. Participates in clinical conferences and in-service programs.
4. Conducts onsite supervisory visits of physical therapy assistants.
5. Advises and consults with the family and other agency personnel as applicable.
6. Prepares clinical and progress notes.
7. Protects and honors patient and coworker confidentiality and right to privacy.
8. Evaluates outcomes of care.
9. Plans for discharge.

Qualifications and Skills:

1. Current Texas PT License.
2. Current Certification in CPR.
3. Valid Texas Driver's License and Automobile Insurance.
4. Knowledge of WellSky EMR software is a plus.

Salary: Pay Per Visit including mileage reimbursement and points assigned on drive time for additional pay.
