We are currently seeking qualified Physical Therapists to join our dynamic team. As a Physical Therapist at Enhance Rehabilitation, you will evaluate and treat patients in a Skilled Nursing Facility (SNF) setting, following an individualized care plan.

Responsibilities include:

• Providing excellent patient care
• Offering exceptional customer service to patients and their families
• Collaborating with facility staff to ensure optimal patient care
• Evaluating new patients and developing personalized plans of care with measurable, functional goals
• Treating patients, addressing concerns, and educating them about their injuries, progress, and outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress and adjusting plans of care accordingly
• Completing timely and accurate documentation, including evaluations, re-evaluations, discharges, and daily progress notes
