Qualifications

• Pharmacist licensure in the state of employment is required.
• 0-1 year of hospital or retail pharmacy experience is preferred.
• Pharmacist degree (B.Pharm or PharmD) is mandatory as stated in education requirements.

Responsibilities

• Accurately prepares, verifies, and dispenses medications in compliance with orders and hospital protocols.
• Reviews medication orders for appropriateness and optimizes therapy.
• Counsels patients and families on proper medication use and adherence strategies.
• Collaborates with physicians and nurses to resolve medication-related issues and improve outcomes.
• Participates in medication safety initiatives, quality improvement, and compliance audits.
• Maintains documentation of interventions and MTM activities in the electronic health record.
