Qualifications

Pharmacist must hold a US-licensed degree; PharmD or PharmD-equivalent required.
0-1 year of hospital or retail pharmacy experience preferred or required?
Active pharmacist license in the state of employment is expected.

Responsibilities

Prepare, verify, and dispense medications in line with prescriptions and regulations.
Review orders and assess patient regimens to optimize therapy.
Counsel patients and families on proper use, side effects, and adherence.
Collaborate with physicians and nurses to resolve medication issues.
Participate in medication safety, quality improvement, and audits.
Develop and implement clinical protocols and guidelines to standardize use.
Monitor outcomes and adjust therapy for safety and efficacy.
Maintain accurate documentation in the EHR.
