Main Responsibilities

1. Evaluate physician medication orders for appropriateness of drug, dosage, and potential interactions.
2. Maintain patient database, including current and past medical history, clinical and laboratory data, and concurrent medications.
3. Ensure appropriate laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with healthcare team members.
4. Obtain and maintain all required documentation in accordance with federal and state regulations and policies.
5. Inform the Pharmacist in Charge of medication-related errors.
6. Perform other duties as assigned.

Minimum Qualifications

1. Bachelor's Degree in Pharmacy.
2. Two years of experience as a pharmacist; long-term care pharmacy experience preferred.
3. Knowledge of principles related to pharmaceutical preparation and practices.
4. Licensed Pharmacist registered with the New Jersey Board of Pharmacy; injection license preferred.
5. Detail-oriented, goal-oriented, team player, with strong oral communication skills.
6. Comprehensive therapeutic knowledge, problem-solving skills, and judgment.
7. Ability to work cooperatively with others and understand current pharmacy practice standards.
8. Proficiency with computer/software applications necessary for job tasks.
9. Excellent verbal and written communication skills.
