Job summary

MEP Health seeks a PRN Nurse Practitioner to provide palliative care in Jackson, TN and surrounding counties. You will evaluate patient eligibility for palliative services, assess needs of patients and families, and coordinate with physicians and interdisciplinary teams to develop and update care plans.

You will conduct regular reassessments, perform home visits, and deliver compassionate, patient-centered care in home settings, partnering closely with the care team to ensure comfort and quality
