Job summary

Tennessee Quality Care Hospice in Jackson, TN seeks a PRN Nurse Practitioner to support the palliative care team. Enjoy flexible, part-time scheduling (15–20 hours weekly) with potential weekend coverage if desired and a well-established employer with competitive pay.

Coverage area includes Jackson and surrounding counties. Qualifications include NP licensure in the state, RN education, CPR, and strong home-visit nursing skills to support patients and families in a home setting.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a Nurse Practitioner in the state of practice.
• Minimum of two years of varied work experience as a professional nurse.
• Must possess current CPR Certification.
• Demonstrated skill in nursing practices and clinical judgments.
• Ability to make home visits and provide nursing care in the home environment.
• Ability to cope with family emotional stress and tolerance of individual lifestyles.
• Interpersonal, verbal, and written communication skills necessary to complete job responsibilities.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Determine patient eligibility for care based on State and Federal guidelines.
• Gather data on physical, psychological, social, and spiritual factors affecting patient/family needs and initiate interventions with the interdisciplinary team.
• Coordinate admission or recertification for care with attending physician and other staff/agencies.
• Utilize resources to meet the patient’s needs.
• Provide regular visits to reassess eligibility, needs, and update the plan of care.
