AccentCare in Seaford, DE is seeking a Nurse Practitioner (Palliative Care) for a PRN/Part-Time role. The NP will develop care plans for patients with chronic and terminal conditions and coordinate with the Medical Director and care team.

Qualified candidates have NP licensure, CPR certification, and the ability to travel between locations. The position offers hourly pay of $55.00-$62.00 with flexible scheduling and comprehensive benefits.

Formación

• Graduated from an approved school of professional nursing.
• One year experience as a Nurse Practitioner is preferred.
• Licensed to practice as a Nurse Practitioner in the state of agency operation.
• Active DEA license in the state where care is provided.
• Possess and maintain CPR certification during clinical role.

Responsabilidades

• Develop and implement clinical plans of care for patients with chronic and terminal illnesses.
• Perform patient assessment, history and physical; recommendations on aggressive symptom-management.
• Monitor patient progress; update leadership and team; coordinate discharge planning and teaching.
• Maintain communication with supervising and attending physicians to review records and care plans.
• Coordinate with other staff and agencies to ensure optimal care and resource use.
• May provide support at time of death.
• Prescribe medication as permitted by state regulations.
