Job summary

Triangle Springs is a Lifepoint Health behavioral health hospital in Raleigh seeking an Outpatient Therapist to provide PRN coverage. You will conduct individual and group therapy, participate in treatment planning, and support family sessions to engage loved ones in care.

As part of the interdisciplinary team, you will coordinate with physicians and nursing staff, maintain documentation per HIPAA, and perform crisis intervention to assure patient safety and smooth transitions after care.

Qualifications

• Master's degree in Social Work or Counseling or equivalent.
• Current clinical or social work license as required by state regulations.
• CPR and De-escalation certification required or within 30 days of hire.

Responsibilities

• Conducts individual and group therapy sessions with patients.
• Involves in treatment planning and active discharge planning.
• Provides family session counseling to involve family and support groups.
• Communicates with clients, families, and outside referral sources.
• Maintains timely, accurate documentation and HIPAA compliance.
• Performs crisis intervention and supports patient safety as needed.
• Collaborates with interdisciplinary team including physicians and nursing staff.
• Assists care coordination and completes all required paperwork.
