Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Greendale, WI. The role supports plan development, implementation, and evaluation of resident OT services.

Full Time, Part Time or PRN positions are available. Applicants must hold an Associate's degree in Occupational Therapy and an active OTA license in the state of practice, with ongoing continuing education to maintain licensure.

Qualifications

• Associate's degree in Occupational Therapy required.
• MUST have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine appropriate intervention or treatment.
• Follow treatment plans and long-/short-term goals defined by supervising therapist.
• Communicate evaluation findings and recommendations to residents and team.
• Establish resident therapy schedules with team members.
• Document and bill occupational therapy services accurately.
• Perform other duties to effectively treat patients as needed.
