Job summary

Aventuraatpembrooke is seeking a dedicated Occupational Therapist to join our skilled nursing facilities in Delaware County, Chester. In this PRN position, you will evaluate residents, develop treatment plans, and implement therapeutic interventions to help them regain independence.

Candidates should have a degree from an accredited Occupational Therapy program and possess a valid Occupational Therapy license. We value compassionate, team-oriented professionals who are committed to providing exceptional resident-centered care.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Valid Occupational Therapy license and registration for the state of practice.
• Experience in long‑term care or geriatrics is preferred but not required.

Responsibilities

• Evaluate referred residents to determine their functional level.
• Develop individualized treatment plans for residents.
• Implement therapeutic interventions to improve residents' skills.
• Complete documentation including evaluations and progress notes.
• Collaborate with the interdisciplinary team.
