Job summary

Creative Solutions In HealthCare is seeking a compassionate Occupational Therapist (OT) to join our in-house rehab team in Odessa, Texas on a PRN basis. You will focus on improving residents' independence, safety, and quality of life through meaningful daily activities and functional outcomes.

Responsibilities include evaluating patients, developing individualized ADL/IADL plans, recommending adaptive equipment, collaborating with the interdisciplinary team, and documenting progress in Net

Qualifications

• Active and valid Texas Occupational Therapy license.
• In good standing with regulatory agencies and boards.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying needs and personalities.
• Strong communication and collaboration skills.

Responsibilities

• Consult with patients to evaluate their physical condition and its impact on daily activities and quality of life.
• Assess and interpret patient evaluations and test results.
• Develop individualized ADL/IADL treatment plans using various therapeutic techniques.
• Assess home environments and recommend modifications to support independence.
• Recommend adaptive equipment and provide training for safety.
• Ensure treatments align with the established plan of care.
• Monitor patient response and modify interventions with the interdisciplinary team.
• Provide clinical education and support to patients, families, and caregivers.
• Collaborate with the interdisciplinary team to ensure high-quality care.
• Complete documentation thoroughly and timely per company policies and regulations.
• Attend required meetings as designated by the Director of Rehab.
