Job summary

Three Oaks Hospice is seeking a passionate PRN On-Call Registered Nurse to join our weekend on-call team. You will provide skilled nursing care in patients' residences, coordinate with the interdisciplinary team, and support families during end-of-life care.

The role requires an Associate or Bachelor's in Nursing, current RN license, CPR certification, and strong interpersonal skills. Experience in hospice or home health is preferred, and proficiency with computer systems and MS Office is

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer is required; Microsoft office suite experience is preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family and referring agency.
• Assume responsibility for coordination of care.
