Job summary

Brushy Creek Post Acute in Greer, SC seeks a compassionate Occupational Therapist (OTR) to join our rehabilitation team on a PRN basis. You will evaluate and treat residents, develop individualized plans, and collaborate with families, physicians, and staff to maximize independence and quality of life.

You will provide skilled OT services, monitor progress, and adjust plans as needed while ensuring compliance with all regulatory requirements.

Qualifications

• Bachelor's degree in Occupational Therapy is required.
• Master's or Doctorate in Occupational Therapy is preferred.
• Experience in skilled nursing, rehabilitation, or long-term care is preferred.
• Experience with PCC and Casamba is preferred.
• Strong clinical, communication, and documentation skills are required.

Responsibilities

• Evaluate residents and develop individualized OT treatment plans.
• Provide skilled OT services according to physician orders and resident goals.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with physicians, nursing staff, residents, families, and team members.
• Participate in care plan meetings and discharge planning activities.
• Document accurately and timely per facility and payer requirements.
• Educate residents and families on treatment plans and home programs.
• Recommend adaptive equipment to improve function and safety.
• Assist in in-service training for facility staff.
• Ensure compliance with policies and state regulations.
