Job Summary
An Occupational Therapist (OT) administers occupational therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and the plan of care under the direction and supervision of the Director of Clinical Services/Nursing Supervisor.
Responsibilities
- Improve or minimize residual physical disabilities of the patient.
- Return the individual to optimum and productive living within the patient’s capabilities.
- Participate with all other home care personnel in patient care planning.
- Provide prescribed occupational therapy.
- Direct and supervise personnel as required.
- Take initial history and make an initial evaluation.
- Perform all skilled procedures.
- Consult with physicians regarding treatment changes.
- Write reports to physicians regarding patient progress.
- Instruct patients and family members in home programs and exercises.
- Complete initial evaluation, plan of care, and goals for physician approval. Submit recertifications every 60 days. Write progress notes after each visit, and reevaluations when treatment duration ends. Write summaries upon patient discharge.
- Present in-service sessions to agency staff periodically.
- Assist physicians in evaluating patient function levels.
- Help develop and revise the plan of care as needed.
- Prepare clinical and progress notes.
- Advise and consult with family and agency personnel.
- Participate in in-service programs and QAPI activities as assigned.
Qualifications
- Graduate of an accredited Occupational Therapy program, certified by the National Certification Examination, or eligible to take it.
- Licensed in the practicing state(s), unless licensure does not apply.
- Preferred: Two years of experience.
Working Environment
Works indoors in agency offices and patient homes, traveling as needed.
Job Relationships
- Supervised by: Director of Clinical Services/Nursing Supervisor
- Supervises: Certified Occupational Therapy Assistant and Home Health Aide as assigned

- Ability to participate in physical activity.
- Ability to work extended periods standing and involved in physical activity.
- Moderate lifting.
- Ability to perform extensive bending, lifting, and standing regularly.
