Job summary

Addus HomeCare is seeking a compassionate Occupational Therapist to join our Home Health team in Dickson, Tennessee on a PRN basis. You will provide one-on-one therapy in patients' homes, developing tailored treatment plans to improve daily functioning and independence.

Responsibilities include documenting progress, communicating with the care team, and retraining patients in activities of daily living as needed. AOTA-accredited program graduate with NBCOT and state licensure is required.

Qualifications

• Graduate of an accredited Occupational Therapy program approved by AOTA.
• Current NBCOT certification and licensed in the state of practice.
• At least two years of clinical OT experience preferred.
• Current CPR certification.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide occupational therapy services per physician's plan of care and state guidelines.
• Develop individualized treatment plans and functional goals to improve independence.
• Communicate care plans with physicians and interdisciplinary team.
• Train patients in activities of daily living and use of adaptive equipment.
• Fabricate splints and train use of prosthetic/orthotic devices as appropriate.
• Maintain clinical documentation and patient status updates.
• Collaborate with healthcare professionals to ensure coordinated care.
