Job summary

Five Star Home Health Care LLC is seeking an Occupational Therapist (OT) PRN to render 1-on-1 treatment in patients' homes in Chantilly, VA. This locally owned agency has over 20 years of home health experience and uses Homecare Homebase EMR for documentation.

The role emphasizes collaboration, education, and training, with a schedule typically Monday-Friday and in-person, as-needed hours. Active Virginia OT licensure is required and prior home health experience is preferred.

Qualifications

• Graduate of an accredited Occupational Therapy program and nationally certified.
• Two (2) years experience providing care as an OT is preferred.
• One (1) year home health experience is preferred.
• Experience with HCHB EMR is preferred.
• Active OT license in Virginia is required.

Responsibilities

• Provide 1-on-1 occupational therapy treatment in the patient's home.
• Use Care Maps to guide treatment for diseases.
• Collaborate with education, training, and other team members.
• Documentation and feedback reviews as part of ongoing care.
