All Care Home Health is a Medicare‑certified home health agency established in 1998. Our mission is to provide compassionate and effective health care in the most comfortable, secure environment for the patient: home. Located in Danville, Virginia, we serve patients of all ages who reside in Danville and Pittsylvania County, and in parts of Halifax and Henry Counties. Our services include skilled nursing, physical therapy, occupational therapy, speech‑language pathology, and home health aide.

We are currently seeking a motivated Occupational Therapist and/or Occupational Therapy Assistant to deliver exemplary care and enthusiasm in working with patients of all ages. These PRN positions are part‑time and offer flexibility to work as little or as often as you want. The role requires travel among patient homes in Danville and Pittsylvania County, Virginia, and you must provide your own transportation.

Job Description

• Assess patient care needs and evaluate level of function.
• Develop and revise treatment plans, preparing clinical and progress notes.
• Advise family and other agency personnel and participate in in‑service programs.
• Support the Occupational Therapist in treating patients, communicating with the primary therapist, and completing documentation.
• Assist in evaluating the level of function, disability, injury, or disease process of individual patients using diagnostic and prognostic tests.
• Provide treatment as ordered to relieve pain, develop or restore function, and maintain maximum performance using therapy modalities.
• Observe, record, and report patient reactions to the therapy program or changes in condition.
• Interpret, plan, and establish proper treatment programs per physician referral, reviewing response and recommending continuation or discontinuation.
• Teach patients, families, and health team personnel regarding therapy procedures.
• Evaluate the home environment and make appropriate recommendations.
• Document each visit and incorporate notes into the clinical record.

Qualifications

• Registered OT, licensed in the Commonwealth of Virginia – or Certified OTA, licensed in the Commonwealth of Virginia.
• Current CPR certification.
• Valid Driver's License.
• No criminal history.
• Experience in Home Health preferred, not required.
• At least one year of experience as a practicing OT or OTA.
