Job summary

Otterbein SeniorLife in West Liberty, OH is seeking a part-time/PRN Occupational Therapist to provide home health services across the greater West Liberty area. You will evaluate, plan and administer therapeutic programs in patients' homes to restore daily living skills and independence.

The role reports to the Therapy Manager/Director of Nursing and requires Ohio OT licensure, CPR, and at least two years of home health experience.

Qualifications

• Graduate of an Occupational Therapy curriculum accredited by the AMA/AOTA; or eligible for NBCOT.
• Eligible for the National Registration Examination of the American Occupational Therapy Association; or two years of OT experience with proficiency exam.
• Current licensure as an Occupational Therapist for the State of Ohio.
• Minimum of two years’ experience as an OT, including home health.
• COTAs supervision requires two years of home care experience and one year supervisory experience.
• Current CPR card and health certificate/TB testing results.

Responsibilities

• Perform initial evaluation and ongoing patient assessments.
• Initiate and update a written plan of care.
• Provide skilled occupational therapy modalities within scope of practice.
• Document care in compliance with regulations and policies.
• Supervise COTAs and conduct regular chart reviews and home visits.
