Job summary

Emerald Care Center Southwest is seeking a PRN Occupational Therapist to evaluate residents, plan individualized therapy, and deliver one-on-one interventions aimed at improving daily living activities and independence.

You will collaborate with a multidisciplinary team, document progress, and educate residents and families while maintaining compliance with facility policies. This role offers flexible scheduling for an as-needed basis.

Qualifications

• OT degree and state licensure are required.
• Experience in skilled nursing or similar setting is preferred but not required.
• Clinical assessment and documentation skills essential.

Responsibilities

• Assess and evaluate residents' physical, cognitive, and emotional needs.
• Develop and implement individualized treatment plans to improve daily living activities.
• Provide one-on-one therapy to address motor, cognitive, and functional goals.
• Document assessments, plans, progress, and discharge summaries per policies.
• Collaborate with physicians, nurses, and other therapists for coordinated care.
• Educate residents and families on rehabilitation strategies.
• Develop and refine therapy programs and protocols to improve outcomes.
• Ensure compliance with facility policies and regulatory requirements.
• Engage in ongoing professional development to stay current in occupational therapy.
