Job summary

Greenridge Post-Acute and Senior Living in El Sobrante, CA is hiring a licensed Physical Therapist on a PRN basis. You will lead evaluations, develop and implement plans of care, and supervise support staff within a collaborative rehabilitation team focused on resident outcomes.

You will provide skilled interventions, oversite of PTAs and aides, and ensure compliance with CA regulations, HIPAA, and facility policies.

Qualifications

• Current, active, and unrestricted Physical Therapist license in California — required
• Graduate of an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate) — required
• Successful completion of the NPTE — required
• Current CPR/BLS certification (or willingness to obtain)
• Experience in skilled nursing/post-acute setting preferred
• Excellent verbal and written communication skills
• Strong organizational skills and ability to exercise clinical judgment
• Team-oriented with independent clinical decision-making

Responsibilities

• Perform comprehensive evaluations and reassessments per physician orders and CA standards
• Develop and implement individualized plans of care with measurable goals
• Deliver skilled interventions, individual and group, to restore function
• Monitor progress and adjust treatments based on outcomes
• Supervise PTAs and aides per CA state law
• Communicate findings and discharge outcomes to physicians and team
• Instruct patients and caregivers on home exercise programs and equipment use
• Complete timely, compliant documentation and participate in care planning
