Job summary

Stonecreek Health and Rehabilitation is seeking a dedicated Occupational Therapist to deliver patient-centered therapy within a collaborative interdisciplinary team in Paducah, KY. You will assess, plan, and provide ongoing OT services, ensure compliant documentation, and contribute to care conferences and program development.

The role emphasizes maintaining licensure, ethical practice, and adapting plans to individual patient needs while supporting facility safety and quality outcomes.

Qualifications

• Graduate of an accredited occupational therapy program.
• Valid state Occupational Therapist license, where applicable.
• Proficiency with Microsoft Office applications and basic internet navigation.

Responsibilities

• Reports to the Director of Rehabilitation.
• Provide screening, evaluation, and treatment to optimize patient functionality.
• Document accurately and timely in accordance with state/federal guidelines.
• Maintain patient-centered interactions with patients, team members, and facility staff.
• Supervise assistants and rehabilitation technicians as assigned.
• Participate in care conferences and interdisciplinary team activities.
• Develop and enhance therapy programs to support quality outcomes.
• Maintain licensure and adhere to payment models and confidentiality policies.
