Sagewood is seeking an Occupational Therapist to join our in-house team serving Assisted Living, Skilled Nursing, and Memory Care residents in Phoenix. The role is PRN with flexible scheduling and a focus on delivering high-quality therapy to a senior population.
Responsibilities include evaluating patients, developing treatment plans, documenting progress, and coordinating care with other staff while maintaining a clean, safe environment and strong resident relations.

An Occupational Therapist professional with a minimum of a Bachelor’s Degree in Occupational Therapy. Sedentary tasks related to job – approximately 10% Physical tasks as listed – approximately 90%. CPR certified. Licensed by the State in the designated geographic area in their respective therapy profession.
