Acacia Health Center at Sagewood in Phoenix, AZ is seeking an Occupational Therapist to provide services in Assisted Living, Skilled Nursing, and Memory Care. The role includes evaluation, treatment planning, and direct patient care on a full-time schedule with flexible weekly options.

Qualifications

• Bachelor’s degree in Occupational Therapy required.
• Arizona licensure required; CPR certified.
• Experience in senior care settings preferred.

Responsibilities

• Maintain accurate patient care records for the caseload.
• Report billing information for Occupational Therapy per patient.
• Provide direct patient care, evaluation and treatment planning.
• Report and interpret treatment results.
• Develop and present in-services for the facility.
• Supervise support personnel as appropriate.
• Conduct screens, evaluations, treatment plans, and delivery per policies.
• Assist with community education and public relations activities.
• Maintain high standard of resident care.
• Respond to crises and calls for assistance.
• Participate in in-services and department meetings.

What you'll do:

• Responsible for the maintenance and accuracy of records in regard to patient care for all patients on personal case load.
• Responsible for the correct reporting of billing information of Occupational Therapy in regard to all patients on personal case load.
• Provide direct patient care as prescribed, including evaluation and treatment planning.
• Responsible for the reporting and interpretation of treatment results.
• Participate in development and presentation of in-services for the facility as requested.
• Supervise support personnel in immediate work areas as appropriate.
• Perform clinical functions: screens, evaluations, development of treatment plans, and delivery of treatment in accordance with facility, Medicare, and Acacia Health Centers policies and procedures. 8. Participate in meetings regarding patient care as appropriate.
• Assist when appropriate with any community education and public relations activities of the facility.
• Maintains a high standard of resident care.
• Responds immediately to all crisis and/or calls for assistance.
• Actively participates in in-services, department meetings and continuing education.
• Provide education to patient’s family as well as staff on the specific treatment plan as well as therapy goals for the patient.
• Responsible for the coordination and cooperation with other staff members in carrying out the treatment plan as established.

Who you are:

An Occupational Therapist professional with a minimum of a Bachelor’s Degree in Occupational Therapy. Sedentary tasks related to job – approximately 10%. Physical tasks as listed – approximately 90%. CPR certified. Licensed by the State of Arizona.
