Nelson Care Center in Alexandria, MN is seeking a PRN Occupational Therapist to screen, evaluate, revise plans of care and discharge planning under physician supervision. The role requires an OT degree and MN licensure, with strong organizational and communication skills.

Ideal candidates will be knowledgeable about Functional Pathways policies and capable of completing required screenings and licensure. This is a PRN/per-diem position within a supportive therapy team in a welcoming community.

Qualifications

• OT degree from an accredited program.
• Active MN state license in Occupational Therapy.
• Excellent organizational and time management skills.
• Excellent written and verbal communication skills.
• Current working knowledge of Functional Pathways policies and procedures.
• Completion of all required screenings, certifications, and licensure; location-specific requirements.

Responsabilités

• Screen, evaluate, re-evaluate, discharge plan as supervised by Physician.
• Make revisions to the established plan of care as needed.
• Collaborate with therapy team to support residents' rehabilitation goals.
