Qualifications

• Bachelor’s or Master’s degree in Occupational Therapy from an accredited institution.
• Valid state licensure as an Occupational Therapist.
• Experience in a skilled nursing facility or similar setting preferred, but not required.

Responsibilities

• Assessment & Evaluation: Conduct assessments of residents' physical, cognitive, and emotional needs.
• Therapy Planning: Develop and implement individualized treatment plans.
• Intervention: Provide one-on-one therapy sessions to address motor skills, cognitive functions, and daily living activities.
• Rehabilitation & Recovery: Use therapeutic techniques and adaptive equipment to improve skills and independence.
• Documentation: Maintain accurate documentation of assessments, plans, progress notes and discharge summaries.
• Collaboration: Work with physicians, nurses, PTs, STs and other professionals for coordinated care.
• Education & Training: Educate residents, families, and caregivers on rehab techniques.
• Program Development: Contribute to therapy program and protocol development.
• Compliance: Adhere to policies and regulations.
• Professional Development: Stay current with best practices.

Job Summary:

As an Occupational Therapist at Emerald Care Center Midwest, you will play a crucial role in enhancing the quality of life for our residents through therapeutic interventions. You will assess, plan, and implement individualized therapy programs aimed at improving patients' ability to perform daily activities and achieve their highest level of independence and functionality. You will collaborate with a multidisciplinary team to provide comprehensive care tailored to each resident's needs.

Responsibilities:

• Assessment & Evaluation: Conduct thorough assessments of residents' physical, cognitive, and emotional needs. Evaluate the impact of impairments on daily activities and functional abilities.
• Therapy Planning: Develop and implement individualized treatment plans based on assessment findings, resident goals, and evidence-based practices.
• Intervention: Provide one-on-one therapy sessions to address specific challenges, including but not limited to motor skills, cognitive functions, and daily living activities.
• Rehabilitation & Recovery: Utilize therapeutic techniques and adaptive equipment to enhance residents’ skills, promote independence, and improve their overall quality of life.
• Documentation: Maintain accurate and timely documentation of assessments, treatment plans, progress notes, and discharge summaries in accordance with facility policies and regulatory requirements.
• Collaboration: Work closely with physicians, nurses, physical therapists, speech therapists, and other healthcare professionals to ensure a coordinated approach to care.
• Education & Training: Educate residents, families, and caregivers on techniques and strategies to support ongoing rehabilitation and daily living skills.
• Program Development: Participate in the development and refinement of therapy programs and protocols to enhance service delivery and outcomes.
• Compliance: Adhere to facility policies, state, and federal regulations, ensuring all practices meet required standards and guidelines.
