Job summary

Anew Home Health in Indianapolis, IN is seeking a licensed Occupational Therapist for PRN home health visits. Therapists will evaluate patients, develop treatment and discharge plans, supervise assistants, and contribute to case management while delivering compassionate care in patients' homes.

Candidate MUST be a licensed Occupational Therapist.

Per-Visit Rates

Rates paid per visit as follows:

• Start of Care (SOC): $150.00
• Resumption of Care (ROC): $115.00
• Reassessment: $70.00
• Routine Visit: $70.00
• SCIC Visit: $95.00
• Evaluation: $85.00

Mileage Reimbursement

• $0.58 per mile

Non-Visit Productivity (i.e. education, staff meetings)

• $45.00 per hour

The Occupational Therapist implements, with minimal supervision, skilled occupational therapy services in accordance with the principles and practices of occupational therapy and within Anew Home Health policies and procedures. Evaluates patients, develops initial and ongoing treatment plans, develops discharge plans, supervises assistants, contributes to case management, and provides quality patient care.
