Anew Home Health is hiring Licensed Occupational Therapists to provide PRN home health services across Indiana, including Fishers and Beech Grove. You will evaluate patients, develop treatment plans, and discharge plans with minimal supervision while joining a close-knit team focused on compassionate care at home.

Qualifications

• Licensed Occupational Therapist required in Indiana.

Responsibilities

Candidate MUST be a licensed Occupational Therapist.

Per-Visit Rates

Rates paid per visit as follows:

• Start of Care (SOC): $150.00
• Resumption of Care (ROC): $115.00
• Reassessment: $70.00
• Routine Visit: $70.00
• SCIC Visit: $95.00
• Evaluation: $85.00

Mileage Reimbursement

• $0.58 per mile

Non-Visit Productivity (i.e. education, staff meetings)

• $45.00 per hour

The Occupational Therapist implements, with minimal supervision, skilled occupational therapy services in accordance with the principles and practices of occupational therapy and within Anew Home Health policies and procedures. Evaluates patients, develops initial and ongoing treatment plans, develops discharge plans, supervises assistants, contributes to case management, and provides quality patient care.
