Job summary

Ambercare Hospice, part of the Addus HomeCare family, seeks a compassionate PRN Nurse Practitioner to join our Las Cruces team. You will provide expert clinical care to patients and families in home settings, coordinating with an interdisciplinary team to ensure comfort, dignity and quality of life.

The role offers flexible PRN scheduling, autonomy to manage visits and caseload priorities, and growth opportunities within a supportive hospice environment.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a Nurse Practitioner in the state of practice.
• Minimum of two years of varied work experience as a professional nurse.
• Must possess current CPR Certification.
• Demonstrated skill in nursing practices and clinical judgments.
• Ability to make home visits and provide nursing care in the home environment.
• Ability to cope with family emotional stress and tolerance of individual lifestyles.
• Interpersonal, verbal, and written communication skills necessary to complete job responsibilities.
• Valid driver's license and proof of insurance is required.

Responsibilities

• Determine patient eligibility for care based on state and federal guidelines.
• Obtain data on physical, psychological, social, and spiritual factors affecting care.
• Coordinate admission or recertification with physicians and teams.
• Utilize resources to meet patient needs.
• Provide regular visits to reassess eligibility and update the care plan.
