Job summary

Premise Health Inc. is seeking a PRN Nurse Practitioner to provide high-quality care to client employees at our Florence Health Center, KY. The clinic operates Monday–Friday, 8:00 AM–4:30 PM, with no nights or weekends, in a patient-focused setting.

You will conduct physicals, develop care plans, educate patients, order tests, and coordinate with physicians to ensure continuity of care. A strong focus on preventive care, documentation, and staff coaching is essential.

Qualifications

• Must hold state RN licenses/certificates to practice.
• Must hold Nurse Practitioner licenses/certificates to practice.
• State Prescriptive Authority (if applicable).
• Federal DEA license (matching each state of practice).
• State Narcotic/Controlled Substance license if required in your state.
• Current national certification (ANCC or AANP).
• Master’s Degree in Science of Nursing, or transcripts if not stated.
• CPR/BLS/ACLS or equivalent Basic Life Support for health care providers; ACLS may be required by contract scope.

Responsibilities

• Conducts physical exams and preventive health measures per guidelines.
• Performs procedures such as injections, suturing, wound care, and infection management.
• Develops patient care plans, records progress, and ensures continuity of care.
• Educates patients on treatment plans, health maintenance, and development.
• Orders and interprets diagnostic tests to assess clinical conditions.
• Collaborates with physicians and healthcare teams for comprehensive care.
• Prescribes or recommends treatments, including medications and therapies.
• Refers patients to specialists or physicians as needed.
• Maintains patient health records with accurate and up-to-date documentation.
• Educates and coaches nursing staff on best practices.
• Performs other duties as assigned.
